Policies and Procedures

Deadlines
Please Note: ALL FORMS & PAYMENT MUST BE SUBMITTED BY JUNE 1. Forms and payments for any registration submitted after June 1 will be due at the time of registration.
Physical Form
Each camper must submit a physical form signed by a doctor within the year prior to the start of their camp session. Any sports, school, etc. form is acceptable, or you may use this form.
Vaccination Record
This year we will require that you upload a copy of the Florida Department of Health’s DH 680 Form - Florida Certification of Immunization, rather than just filling it out within the Medical Form. This is the same form required for schools and other childcare facilities within the state of Florida. You can request this from your healthcare provider when you go for a physical, visit your local health department, or request this form online by following the instructions here.
We know a lot of you upload this with your Physical Form already, so this will simply be a separate location for you to upload to.
